Traditional Home Care (Habilitation Division) has an immediate opportunity for a DSP Coordinator in our Pottsville Office.
The ODP Care Coordinator will oversee assigned caseload, staff, and provide direct services to individuals with intellectual and developmental disabilities. Prior experience in IDD Waiver services and/or employment services preferred. Experience in ID compliance, case management, HCSIS and EIM, staff supervision including 24-7 on-call staffing, and activity planning preferred. This position will help people who live at home by promoting independence, working on personal goals, helping the individual participate in community activities, assist in job finding, and various other daily tasks. The position is expected to meet and exceed budget and client growth projections.
Duties and responsibilities include, but not limited to the following:
• Manage assigned caseload of clients and staffing cases appropriately.
• Obtain training/certification according to ODP and agency requirements and Inclusive Academy training programs. Promote said programs to Supervisors, DSPs and Job Coaches.
• Leading assigned caseload, including assisting in scheduling, recruiting, hiring, and supervising assigned caseload staff.
• Maintain positive relationships with clients including informing clients of agency activities, notifying clients of schedule and staff changes.
• Conduct and document monthly 'touch base' meetings with clients / families, including service satisfaction reviews, and other pertinent information.
• Ensure compliant case management for ODP systems, including maintaining updated files and other compliance matters as directed.
• Attend agency meetings as requested including ISP, Intake meetings, and others as necessary and file all paperwork properly as required.
• Other related duties as assigned.
Qualifications:
• High school diploma / Bachelor's Degree
• Positive, enthusiastic attitude and professional appearance
• Excellent communication, presentation, and social management skills, computer knowledge, strong documentation and organizational skills.
• Valid PA Driver's License, clean driving record, and willing to travel
• Ability to work independently without constant supervision
• Must Pass Child Abuse History and Criminal Record Check Clearances and not be excluded from State of PA Exclusion Lists
• Experience working with the Behavioral Health/Intellectually Disabled Population
• Office of Developmental Programs experience - 1 year preferred
